How do I establish my child’s care at Child & Adolescent Specialty Care?

You can call our office at 937.667.7711 and tell us you would like to transfer your child’s care to CASC. We will get all the needed information from you and set up your first appointment if your child is due for a well check. If you are about to have a baby, you can call to arrange a prenatal meeting with our providers to establish your newborn’s care with us. Please note that patient records need to be on-site before appointments can be made.

Are you accepting new patients?

Yes we are accepting new patients as long as:

  • We are contracted with your health insurance provider

  • You follow the recommended vaccine schedule by the CDC

Do I need to attend all of my child’s appointments?

For sick appointments, any adult that you designate may bring your child. Please send a note with them or call to provide verbal consent.

For well visits all children under the age of 16 are required to be accompanied by a parent or legal guardian. Children 16 and older may attend without a parent but require a form to be completed prior to the visit giving consent for vaccinations and treatment without a parent present.

How do I get urgent medical advice for my child after hours?

During business hours, you can call our clinic and speak with one of our nurses. For issues that can’t wait until business hours, we offer after hours assistance at 937.667.7711 (option 9). They can provide helpful advice to get you through until our office opens for appointments, or direct you to an urgent care or emergency room if indicated by your child’s symptoms..

Do I have to see the same provider each time I come to the office?

No, you may see a different provider with each visit, or you may choose to see a specific provider as often as possible. We do our best to accommodate the preferences of our patients and their families.

If I need to have my child seen after hours, where should I go?

If your child requires immediate care when our clinic is not open, we have provided a list of our recommended after care facilities for your convenience. We believe the providers there can provide optimal care in an emergency when Child & Adolescent Specialty Care is not available.

How can I get a copy of my child’s medical record?

You may request a copy of your child’s medical records (in part or in whole) in person at our office. The fee for this is $15 per request.
